Box 4768 / Church Street Station New York, NY 10261-4768 You can fill in and print a copy of the New York City Dog License application through the New York City Department of Health website. You'll still have to mail it in with supporting vaccination documentation, proof of spay/ neutering certificates, and you check for the appropriate amount. Processing could take from 4 to 6 weeks.
